​The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs.

PACE at Hudson Headwaters’ growth, progress and success depend on our staff – people with initiative, creativity, enthusiasm, and compassion, working in an atmosphere of mutual respect. Each staff member is an important member of our team and directly contributes to our mission - to promote and sustain the independence of older adults with significant health needs who want to continue living safely at home and in the community.
