Live within the PACE service area (parts of Saratoga, Warren, and Washington counties)

Certified by NYS as meeting the need for nursing home level of care
-
Able to live safely in the community with the support of PACE services

Payment Information
Most PACE participants are dual-eligible, meaning they qualify for both Medicaid and Medicare although there are also private pay options.
PACE at Hudson Headwaters has an approved contract with the Centers for Medicare and Medicaid services (CMS) and New York State Department of Health (DOH). Enrollment in PACE at Hudson Headwaters depends on renewal of its contract with CMS and DOH. Participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.

Upon enrollment, our interdisciplinary team completes an in-depth healthcare evaluation and develops a comprehensive, individualized care plan based on medical, physical, emotional and social needs. The individual’s enrollment becomes effective of the first of the month.
